"As much as Isaac would like to have a friend in this dark and terrifying new reality, he had already been hurt so much by those who seem friendly at first."

This mod contains the following tropes:

  • Arrange Mode: The mod will eventually add "Tainted" versions of most songs, named after the secret characters from Repentance.
  • Beam-O-War: Girlfriend is engaging in one with an incubus in the background of "Decide".
  • Body Horror: Delirium, instead of changing form completely, has melting heads from Daddy Dearest, Mommy Mearest, Pico, Isaac, Monster, Tankman, Skid and Pump pop out of its body.
  • Camera Perspective Switch: "Delirious" is shown in first person, with Delirium staring directly at the screen.
  • Cover Version: "Sacrificial" and "My Innermost Apocalypse" are covers of songs with the same name from the flash version of Isaac, but with vocals.
  • Demoted to Extra: Isaac's mom only appears as a cameo that occasionally appears in the main menu, along with voice only appearances in the intro cutscene and the cutscene before "Decide".
  • Dual Boss: Inverted in the Tainted remix of "Decide", which has Boyfriend and Girlfriend face the demonic Isaac together.
  • Earn Your Bad Ending: The Evil Route's two songs are far harder than the equivalents on the Good Route, with "Decide" in particular being one of the hardest songs in the mod.
  • Flavor Text: Each song is given a description in the Freeplay menu:
    Sacrificial: First contact.
    My Innermost Apocalypse: Ascend.
    Misperception: Corrupted by sin.
    Decide: My true self.
Acceptance: And then… he saw nothing.
    Avarice: Give me a voice.
Delirious: It's all in the mind.
  • Flipping the Bird: Dr. Fetus gives a middle finger in his left and up poses. Not surprising, considering it's Dr. Fetus we're talking about.
  • Hanging Around: Greed is dangling from a noose during "Avarice", swinging back and forth during its idle animation.
  • Interface Screw:
    • The Curse of the Unknown is in effect during "My Innermost Apocalypse", which, just like in Isaac, prevents you from seeing your health.
    • The Curse of Darkness will occasionally trigger during "Decide", turning the screen black. Hitting the Brimstone notes will cause the note chart to be covered with a Brimstone beam.
    • Isaac's drawings will occasionally cover the screen during "Acceptance".
  • Losing Your Head: Greed's up pose has his head temporarily pop off.
  • Luck-Based Mission: The pills that appear in "Avarice", just like Isaac proper, have random effects. These range from beneficial things like fully healing you (Full Health) or slowing the scroll speed (I'm Drowsy…), or bad things like damaging you (Bad Trip) or speeding up the scroll speed (I'm Excited!). Downplayed if you play the song on Easy, as while the pills are still random, they're all good effects because Boyfriend is holding a PHD.
  • Mythology Gag:
    • Each song begins with a pop-up telling you where the song takes place with the song name underneath it, much like in Isaac.
    • In the TBoI, Delirium takes the form of all previous bosses fought in the run as part of its boss fight. With the forms of the Dearests, Pico, Monster, Tankman and the Spooky Month kids appearing in Delirium. Aside from Isaac — who's fought in this mod by default — that's all the opponents BF had fought in the base game demo.
    • At one point in "Delirious", the song takes leimoteifs from "Misperception" and "Decide", with the scenery suddenly changing to the Basement and the Dark Room respectively. In TBoI, the Void is an Eldritch Location that constantly changes appearances to those of old locations.
    • "Acceptance" is given the Flavor Text of "And then… he saw nothing", a quote from the true final ending of The Binding of Isaac: Repentance.
    • "Acceptance" also has a Funny Background Event of Girlfriend looking rather unamused while next to a chest containing Bob's Brain, an item from The Binding of Isaac that's infamous for being rather unwieldy.
    • "Avarice"'s Flavor Text references the Alternate Reality Game that unlocked the Keeper in Afterbirth.
    • Dr. Fetus' song, "Raptured", is named after Chapter 5 of Super Meat Boy, "The Rapture", where Dr. Fetus causes The End of the World as We Know It. The song also has a voice say "P-P-Push the buttons!" before the beat drops, referencing what the narrator says after inputting a Cheat Code in that game.
  • Precision F-Strike: Pico lets a deadpan "fuck" halfway through "404", and ends the song with "I'm getting too old for this shit".
  • Sad Battle Music: Both versions of "Misperception" are pretty sad sounding piano pieces, the Tainted version even more so.
