  • Acting for Two:
    • Shannon/Fujika and Stephanie/Chinatsu both share a voice actress in the English (Lee Kitts) and Japanese (Tomiko Suzuki) versions. Subverted in the Drama CD, where Shannon is voiced by Mieko Nebashi and Stephanie is voiced by Chiaki Maeda.
    • Nurse Cook/Kishi and Kathryn/Yayoi share their voice actresses in both the game (Taeko Nakanishi) and in the Drama CD (Makiko Mizogami).
    • Allen Hale/Takashi Midōshima and Henry Kaplan/Tatsushi Uro share their voice actors in the Drama CD (Ginzō Matsuo).
    • Akio Ōtsuka voices Doug Bowman/Wataru Gōmoto, George Maxwell/Fushito Saidō AND Henry Kaplan/Tatsushi Uro in the game.
  • B-Team Sequel: Clock Tower's creator and the director of the first two games, Hifumi Kono, felt he had told all the stories in the universe he wanted to tell with those games and declined making another one. With those games having sold fairly well, however, Human Entertainment, wanted to continue the series, and as a result Yutaka Hirata was instead made the director and designer on Ghost Head/The Struggle Within.
  • No Export for You:
    • PAL territories never got the game.
    • The Drama CD and the guidebook aren't released outside of Japan.
  • The Other Darrin: With the exception of Kae Araki as Alyssa/Yu, none of the other voice actors from the game reprise their roles for the Drama CD:
    • Bates/Sho: Houko Kuwashima replaces Fujiko Takimoto.
    • Alex Corey/Hitoshi Ishizue: Naoki Kinoshita replaces Issei Futamata.
    • Doug Bowman/Wataru Gōmoto: Yoshiyuki Kono replaces Akio Ōtsuka.
    • Allen Hale/Takashi Midōshima: Ginzō Matsuo replaces Tamio Ōki.
    • Shannon Lewis/Fujika: Mieko Nebashi replaces Tomiko Suzuki.
    • Philip Tate/Hajime Takano: Shinobu Satouchi replaces Kōji Yada.
    • Stephanie Tate/Chinatsu Takano: Chiaki Maeda replaces Tomiko Suzuki.
    • George Maxwell/Fushito Saidō: Tomotsu Nishiwaki replaces Akio Ōtsuka.
