In the prologue, O.S.A. agent B.J. Blazkowicz and the british Agent One are sent to the egyptian town of Ras el-Hadid. After destroying all the radio equipment of the Nazis, they make their way through the city, eventually reaching the Nazi headquarters and obtaining information on the excavation done in the area. They then make their way through the catacombs, and are noticed of the presence of SS head operative Helga von Bulow. After disposing of Helga's elite guard and reaching the airfield, they escape in an airplane, only for said airplane to crash near Castle Wolfenstein, where they're detained.

Agent One perishes after Doctor Zee's interrogation, so Zee sends a guard to get Blazkowicz. Unbeknownst for the guard, Blazko was awaiting for him and takes him down. Then he makes his way to Zee and takes him down too. He dispatches the rest of the guard stationing at the Castle and manages to escape via a tram connecting the Castle with the city of Wulfburg, where Kessler, an informant in the Kreisau Circle, was awaiting for Blazko upon hearing his escape.

Then Kessler informs Blazko of the situation. Blazko infiltrates the catacombs... which turn out to be haunted, as ancient warriors awaken and start chasing the Nazis alongside Blazko. He eventually reaches the defiled church, where von Bulow and Professor Zemph are conducting a series of experiments, and they eventually discover the hidden power of the Dagger of Warding, to which Zemph advises to be careful on manipulating it, getting shot by the impatient Helga. The Dagger eventually awakens Olaric, an undead monster that consumes Helga, but Olaric is no match for Blazko.

Once safe and recovered, Blazko is sent in a new mission: infiltrate a secret Nazi operations base in Katamarunde in order to shut down a rocket containing a deadly virus, the V2 rocket, whose operations are overseen by one Wilhelm "Deathshead" Strasse. The mission is a success, and the Nazi guards sent to dispatch Blazkowicz weren't enough to stop him. Before he leaves, he takes down a big Radar. Then he heads to the airbase and prepares an Aircraft in order to escape to the O.S.A. headquarters in Malta.

The next step has Blazko rescueing a defecting Nazi scientist in Kugelstadt and pursuing Deathshead into his secret weapons facility. He eventually discovers a project book called "Venom Gun", a weapon capable of shredding targets to pieces. Deathshead manages to escape, however, B.J. eventually gets the location of Deathshead's secret base in Norway.

So B.J. gets dropped outside of Deathshead's labs in Norway, where he makes his way, gunning down Nazis and strange creatures, until he reaches Deathshead himself. Deathshead, however, doesn't face Blazko one-on-one. That's a job for his newest creation: the Ubersoldat... which still is no match for B.J.. Completely defeated, Deathshead manages to escape to safety via rocket.

After studying the documents rescued by B.J., the O.S.A. learns of Operation: Resurrection, a plot to revive Heinrich, an old ruler of Europe who was contained within a tomb by a monk during the Dark Age. So B.J. is dropped in the Bramburg Dam and makes his way to the village of Paderborn, where he stealthily kills five high-ranking members of the SS. Then he enters into the Chateau Schufstaffel until he reaches the backyard where the anointment ceremony is taking place. Being too late to stop the ceremony, he's left facing two Ubersoldats.

However, not all's lost, as he discovers an alternate path into the Nazi excavation site that leads him to return to Castle Wolfenstein. He eventually reaches the revival site for Heinrich, and bests him after a hard-fought battle, destroying Heinrich in front of Heinrich Himmler himself.
