Directed by: Juan José Campanella

Teleplay by Thomas L. Moran and Peter Blake based on a story by Moran

A woman wins a drag-race. But as Casey's being interviewed, she starts feeling woozy, then falls down.

The applicants are pondering which case to take next, but Dr. House has already decided: the race car driver, who House nicknames "Speed Racer." The applicants have exotic potential diagnoses, but Dr. Foreman's convinced it's just dehydration and heat stroke.

A CIA agent (Chad Willett) comes in asking for Dr. House, who pretends to be someone else. But the agent has a photo of the House. The applicants think maybe the guy's a cop who's been sent to apprehend House. Actually, the agent wants House to consult with Dr. Samira Terzi (Michael Michele) on the case of an agent who may have been poisoned in Bolivia or Brazil.

Dr. Cuddy of course thinks that House's CIA consulting assignment is just an excuse to get out of clinic duty.

Dr. Brenner confesses to something but House won't fire Brenner, Brenner must quit.

Cuddy still refuses to believe House consulted for the CIA. House makes up a story about helping a rich hedge fund manager's son with the sniffles in the Hamptons. Cuddy briefly pretends to believe that, before declaring that the Hamptons story is even less plausible than the CIA story. House must now do 24 clinic hours, relieving Dr. Wilson of 16 clinic hours.

This episode contains examples of the following tropes:

  • "Blind Idiot" Translation: Agent Smith translated "castanhas do Pará" literally as "chestnuts from Pará", not realizing that's what brazil nuts are called in Brazil. This is of extreme medical importance.
  • Shout-Out:
    • To 24: When Dr. House arrives at the CIA medical facility, he says it looks "better" on 24.
    • To Speed Racer: Dr. House nicknames the patient "Speed Racer" because she's a race car driver.
  • So Crazy, It Must Be True: Even though Dr. House is a world-renowned diagnostician, both Dr. Wilson and Dr. Cuddy are skeptical that the CIA has called on him to consult. Wilson is only convinced after calling the Langley switchboard. But Wilson forgets the phone number and so Cuddy assumes Wilson is covering for House. When House gets back from the CIA and realizes Cuddy doesn't buy it, he makes up a story about helping a rich guy's son who just had the sniffles.
  • Well-Intentioned Extremist: Brennan overdosed Casey with Valium to fake Polio and cure her with vitamin C so he could prove such treatments were worth researching.
