In "It Came From the Fan Club", Dinko presents Tommy's skeleton. Tommy's skull is noticeably cracked, implying he suffered a severe head injury (and possible brain damage) that never fully healed. In "They Took Tommy's Brain", Tommy got repeatedly hit in the head, especially when Gumpers made him repeatedly jump off the lighthouse head-first.
