Fridge Brilliance

• The entire game is a play and the player is an actor: whenever you get immersed, you are getting immersed into the role of the healer and giving a better performance.
• The sound effects that play when reputation goes up or down — a boy cackling and a girl crying, respectively — could plausibly be the voices of the Powers That Be, who are revealed to be a boy and a girl playing make-believe with the town and characters. The Changeling's route explains that they sometimes disagree about the scenario, with the girl being more idealistic (seeing Clara as a healer) and the boy more sadistic (playing Clara as a monster) — this would fit with the genders of the two sound effects. The girl is upset when you do bad things, and the boy sneers when you try to do good things.
• Similarly to the above: Every generic NPC is represented with a rag doll for their Character Portrait. Seems like a weird choice at first, but it's foreshadowing for the Powers That Be seeing everyone in the town as a doll.
