* The premise alone is equal parts tragic and horrific. Something that happened four years prior was so horrific and traumatizing that a friend group was split apart, one of said friends has become a nervous wreck that barely functions on a day-to-day basis, and another has almost ''ceased to function'', shutting themselves away and retreat into delusions based on their childhood. Putting aside the more fantastical aspects of Headspace, the core of the game's premise is [[RealismInducedHorror horrifyingly realistic]].
* '''Something'''. This strange ''thing'' haunts you throughout the entire game. Sometimes it's just out of the corner of your vision, sometimes it appears [[FreezeFrameBonus for just a split second]], and sometimes it appears out where you'd least expect it to be. It's always there, it's always watching you, and it's a constant reminder that things are never as innocent as they may seem.
** The appearance of Something is slightly unnerving; it's an abstract shape with a single unblinking eye that stares almost accusingly at you. What makes it go from unnerving to horrifying is the reveal of what that shape ''is''. It's an abstract interpretation of ''a hanged corpse'' staring at Sunny through its hair.

** Most rooms in Black Space have [[StylisticSuck unnerving, hastily-scribbled]] drawings, realistic depictions of random animals, skeletons, and items all over the place that clash heavily with one another, as well as the rest of the game's art styles. There are absurdly tall scribbled stickmen, abstract hanged bodies, massive blobs that only passingly resemble anything, and the occasional photo-realistic image juxtaposed against the chaos. Among them, you can spot facsimiles of Omori as he appeared in the ''Omoriboy'' webcomic, albeit warped and made damn-near unrecognizable.
** Within one of the rooms, a massive Mari head can be found with [[BodyHorror her brain sticking out of her head]], constantly telling Omori to expand his mind. As you keep talking to her, her brain ''grows'' until it crushes her.

She went to talk to him, and he didn't want to discuss it, in a haze that he wasn't good enough if you can uncover the hidden descriptions. Sunny tried to walk away from her; when Mari wouldn't let him leave, he pushed her down in a fit of anger. Unfortunately... she was right next to the staircase. Cue a photograph of Mari's crumpled body over the remains of the violin with her neck snapped. Sunny then [[MyGodWhatHaveIDone panicked]], ran downstairs, and [[PleaseWakeUp begged her to wake up]]. After apologizing for what he did, he puts her to bed. Sunny was all alone in the house, with neither of their parents in sight.

As you turn back, Something suddenly appears, forcing you to run back the way you came. As you reach the entrance of the hallway, you get [[JumpScare brief flashes of Basil and his Something looming over him]], before being sent back to White Space (where you have to stab yourself again to wake up).

When you sacrifice the Stray Cat, the room abruptly inverts its colors as it spawns five Hush Puppies, which are poorly-drawn dog faces screaming to die, and the silly BGM gets swapped to the same one used in the shadow fight. The effect fades away after you've killed the Hush Puppies, but it certainly is a MoodWhiplash.

** One of the doors will take you to a small room with Omori's cat Mewo lying on a table, surrounded by various sharp tools. The only other NPC in the room says that Mewo has been "very bad," and one of the only things you can do there is selecting "Yes" when the game asks if you'd like to cut Mewo open. What's worse is that you're required to select it repeatedly. You get to see Mewo go from being mildly curious as to what you're doing, to [[OhCrap struggling desperately to escape out of sheer terror upon realizing what's about to happen to her]].
