Unfortunately for Randoh, the surgeon who did it modeled his new face on the photograph in his pocket - a photograph of Randoh's unrequited crush, Kurimi Rina. While trying to fix matters, he runs into Rina who mistakes him for her twin sister Yuna, who ran away from home some time ago. Not wanting to cause her family more pain, Randoh decides to live with crush's family until he can 1. get a picture of the old him so the doctor can fix his face, and 2. find the real Yuna.

* FridgeLogic: Invoked. The alternate ending was to have [[spoiler:Randoh's actual family revealed as getting surgery from Manabe and winding up as Kurumi's family, but the creator dropped it for this trope when he released he'd leave too many plot thread hanging ("Rina" being able to deceive her friends, Randoh's parents knowing about Yuna's run away, blah blah blah..]]

* SurprisinglyRealisticOutcome: [[spoiler: The real Yuna is understandably pretty pissed off to discover a guy has been impersonating her and living with her family.]]

* YouCantGoHome: Randoh’s first instinct after waking up is to try and go home, only to find out his family had moved away since the accident, having assumed he was dead.

* PantyShot: Thankfully, ''none'' from Randoh. (You would have gotten one in a cut chapter.) Natsuo, on the other hand, has the accidental panty shot as her curse in life.

* SchoolgirlLesbians: Nozomi is unflinching about her affection towards Randoh/Yuna (and later, Natsuo; she calls both "OneeSama", too), even when confronted about its abnormality by other students. Also, Keiko DoesNotLikeMen, but even she's creeped out by Nozomi.

** Randoh has an ImagineSpot when he first meets [[PsychoLesbian Nozomi]] in the style of ''[[LightNovel/MariaSamaGaMiteru Mari Mite]]''. With roses and sparkles and everything.

* EasySexChange: Dr. Manabe constantly tries to pitch full sex-change surgery to Randoh. Randoh knows better. Heck, Randoh wakes up just as Manabe was about to perform one on him.

** The plot is kicked off when the surgeon assumes that the photo Randoh had was of himself. How many people (aside from extreme narcissists) carry pictures of themselves? In all fairness, Manabe is a total idiot.
* GenderBender: Randoh... technically. He keeps his gems and pecker, but his body mass slimmed down to insanely girly lengths, and his hair grew quite long.

** To ''Manga/GalaxyExpress999'' in a short joke, and ''TheRing'', in the form of the ScaryStalker [[DepartmentOfRedundancyDepartment stalking]] Manabe.

* MagicPlasticSurgery: Thankfully, the author didn't [[YouFailBiologyForever fail biology]] and pointed out the dangers of multiple facial surgeries.

** Even though only Rando's face was changed by the surgery, he is way too cute for a guy...
