* ''VideoGame/TheWindRoad'' has one of the bosses, Qu Wuxie, who looks more like a young woman than a dude given his flowing long hair, effeminate voice, shapely body and feminine-looking body concealed by robes. In his first cutscene he doesn't have any dialogue and players could very well mistook him for a lady, but when confronted as a boss his voice is a man's. Also, calling him "pussy" or "sissy" is a BerserkButton guaranteed to tick him off.

* Canute in ''Manga/VinlandSaga'' is initially mistaken for a girl once he removes the helm hiding his face, and though the confusion gets cleared up fairly quickly, Thorfinn continues to use the insulting nickname "Princess" for a bit, and Thorkell's men once make a crack that if Thorkell is the avatar of Thor, Canute must be the avatar of Freya.

** Neji Hyuga is noted to be pretty as a girl, and having the same face as his cousin Hinata in the spin-off series {{Rock Lees Springtime of Youth}}. It helps that [[{{LongHaired Pretty Boy}} his hair falls to his waist]].

** If we include the spin-off [[{{Rock Lee's Springtime of Youth}} Naruto SD]], then Neji Hyuga too. It’s commented that he has the same face as his cousin, Hinata, and [[{{Long-Haired Pretty Boy}} he wears his hair long]] too.

* ''[[Anime/{{Patlabor}} Patlabor: The Mobile Police]]'': Baddo is a 12 year old boy from India, who looks so feminine that he could be a dead ringer for [[Anime/NadiaTheSecretOfBlueWater Nadia]]; albeit with longer hair. [[AffablyEvil Richard Wong]] used it to his advantage at the end of the Griffin arc, by disguising Baddo in feminine Indian garb, to cover their escape.

* Fierro of ''VideoGame/EternalEden'' fell in love with Jean despite Jean telling him that he's a man.

* ''VideoGame/InazumaEleven'': Madoka is explicitly mentioned to often be confused for a girl. He wears earrings, has a feminine bob hairstyle, and has prominent eyelashes. His name is also a GenderBlenderName.
** Even in ''Anime/InazumaElevenGo'': Kirino has feminine eyes and a rose pink hair tied into two pigtails. He is actually a boy, so don't be fooled be his appearance.

* Narancia Ghirga from ''Manga/JoJosBizarreAdventureGoldenWind'' certainly qualifies, being a skinny teenage boy in a sleeveless black attire with neck straps, an orange skirt, a pixie-like medium-length hairstyle and a rather feminine voice. (Amusingly, his [[FightingSpirit Stand]], [[MusicalThemeNaming which are often named after music references in the series]], is named Aerosmith, known for the song "Dude Looks Like A Lady")
** Narancia's friends, Bruno Bucciarati and Leone Abbacchio, with their androgynous faces, feminine haircuts and Abbacchio's use of makeup, also occasionally get new viewers misled, though they at least sound clearly masculine.

* Kazumi in ''LightNovel/GargoyleOfTheYoshinagas''. That he looks like a girl is one of the things his little sister teases him about to no end. Still, he's generally a nice guy and even has a girl that has a crush on him.

* Yuuta of ''LightNovel/LoveChunibyoAndOtherDelusions'' looks quite feminine according to a few characters and, in the light novels, himself.

* Totsuka Saika from ''LightNovel/MyYouthRomanticComedyIsWrongAsIExpected''. The adorable tennis club member is noted to be cuter and more feminine than most actual girls by the protagonist narrator.

* Shun/Yun from ''LightNovel/OnlySenseOnline''. In real life, he looks so feminine that, when he was once forced into competing in a beauty contest, he was the winner. In the titular game, he's been assigned a female avatar, due to the system mistaking his actual gender.
