The problem here is that the description is reaching in too many directions. In reality, the description is claiming a little bit of all the other misnaming tropes. So the only solution I see is making this more narrow and giving some actual explanation for this trope.

The trope should be specifically about people who dislike being referred to in an informal manner. And just so this doesn't turn into PeopleSitOnChairs, it has to address why! Why does the character even care? What are the writers trying to say about them? What traits are common in all these people?

Namely, it's the want of respect, and sometimes a feeling of entitlement, that should define the character to whom this trope applies. These types of characters have worked hard to get where they are, or have been around the block, and they don't want to be insulted by subordinates, students, young people, or ignorant people.

This trope serves to reinforce their status in society, a group atmosphere, or a bureaucracy. It's shorthand for "I mean business, and you are beneath me; don't forget that."

Examples:
* In "WesternAnimation/TheBoondocks" "A Pimp Named Slickback" prefers to be called by his full name, which is all of those words. Not just "Slickback" or "Mr. Slickback," but "A Pimp Named Slickback."

This is what this trope should really be. Otherwise, it is redundant. As it, it'd be a MTD for me as well.
