Do the descriptions really need to all be hidden under notes? If they were longer it would be a good way to avoid clutter, but most of them are just one line, so it seems to hurt readability more than it helps.

I think the usual Trope: Description format makes the most sense. Any objections if I change it?

None of the fail forever tropes are on here. Does the index fail at being a fail index?

There are no You Fail X Forever tropes anymore. They've all been renamed to Artistic License and are on the Artistic License – Indexes.

A better name might be "Rule of Fail".
