One of them is involuntary and the person experiencing it often thinks it's real. The other is usually voluntary, and the person almost always knows it's a simulation. Not understanding the difference between those two is a problem I can't help you with.

How about we make the definition "Seems like a Batman Cold Open, but it's revealed that the characters were never really in any danger, or that any danger was only for the sake of practice."
So that would include the whole range: training simulations, training in general, and dreams.
So basically the trope would be Fakeout Batman Cold Open.

Plenty of times you know you're dreaming, and people develop controlled dreaming as a skill.
All Just a Dream is any time the apparent situation is not reality. That currently covers training role play games, holographic simulations, and dreaming.
