Two transformations is fine. There's no real minimum number we need to adhere except that a identity crisis of some sort has to happen.

Hm. The quote and the image both demonstrate “Loss of Identity caused by shapeshifting”, but the description says “shapeshifter’s mental distress causes them to start shifting randomly”.
That sounds more like Self-Perception Shapeshifting or Shapeshifter Swan Song.
Could be just a variant of this that results in Power Incontinence.

The article for Shapeshifter Identity Crisis doesn’t quite illustrate the trope. It only shows two form changes, one of which only partially changes the shapeshifter. I think that particular Brawl In The Family comic is a great image choice, but the image should be cropped differently so at least one other transformation is included.
