The tools arrayed include various scissors, surgical knives, pliers, and other handheld torture implements, several of which have blood caking and rust. You also see a large, wide, saw stretched along the back half of the table, with a handle on each end. There also seems to be a few small jars resting near the corner.

You slope the walls of the tunnel entrance such that you can easily climb out. The area immediately around you seems to be clear. As mentioned before, most of it is forested, with lights from the city of Vus lying to the east.

The two of you look into town, though there does not seem to be much that is different from before.
As mentioned before, much of the town is surrounded by a short stone-built wall, with a few key avenues going in. These appear to be located near the beach, leading to a cobblestone-paved market area, and two more further north. The ones northward are only narrow enough for a few people to go in and out at a time, and the torches placed on the outside of the walls have a greater concentration near here.
The market area leads into a semi-circular harbor area, with several buildings sitting on the outer rim. A few guards seem to be positioned near this area, while one patrols in the center, going around a crumbled stone mound.
Although you cannot see much inside the city aside from the rooftops and lights, due to the tree cover above, there seems to be more activity than there was before.

You acquire some of the rusted surgical knives and step out of the cell. The hallway goes on for a few yards to your left but then runs into a dead end. Down the hall to your right, you see a number of cells lining each side. The hall eventually turns to the right, leading to the stairway to the main throne area. There does not seem to be anyone around.

You pass through the corridor. Most of the smaller cells have similar furnishings to the one you were in when you were first captured. Most of them seem to be empty, save for one gaunt man sleeping in one of the cells.
As you reach the end of the hall, you begin to turn, and start ascending the steps. A hand creeps out from around the corner of the exit up ahead, and you see an eye peek out from the left side.
“What the…” you hear the guard up ahead speak aloud.

The direct paths into the city have already been mentioned to you. You could circle around the city to see if there are any other ways in, but it is unlikely you’ll find much that is different from this side. Most of the city area, from what you can see, is covered in cobblestone streets, or is obstructed from sight. Judging from the distance you went while digging, you had managed to tunnel underneath the city, at least a few dozen feet or so past the outer walls. No matter how hard you look, you will not find any easier methods of accessing the city from your current position.
Also, it should be noted that you cannot readily determine where the guards are positioned, save for the ones you can see within the harbor area and the ones nearby who occasionally poke their heads out from behind a few of the buildings, many of which stretch just a bit higher than the walls around them. While the torches outside the wall might suggest where they are positioned, you cannot tell with 100% certainty whether each one corresponds to the position of a given guard, as a few of the guards that peek their heads up are positioned a bit away from them. It should also be noted that these walls aren’t really wide enough to be walked on, and there do not appear to be any guards walking on them. They do not have crenelations either, as they’re relatively simplistic in construction.

Just to be clear, how close am I to the guard?

Probably close enough to be within telekinesis range, though you'll have trouble maneuvering stuff past the exit unless you get close to it, due to the walls/ceiling blocking your direct line of sight.
