Ninja Gaiden 1 for the NES I've never been able to beat on actual hardware fair and square. Something about the bad respawn coding and that bullshit "Die at boss, start ENTIRE BLOCK ALL OVER AGAIN" stuff never sat well with me.

The Dark Worlds of Super Meat Boy.
The main story ramps up from "challenging" to "fairly hard" while the Dark Worlds get more mindblowingly insane as you complete them.

I've still never gotten very far in the first Zelda game for the NES. There've been other games I never beat because of Rage Quit too. I guess I prefer games that have a medium difficulty, if the payoff doesn't seem worth the high blood pressure and possible damage to my controllers, it's time to shut it off and sell the game (unless it's the first Zelda game, which is too cool to sell off even if it is insanely difficult ^^;)

However, using a guide for PSII is completely legitimate. The game came with one!
Zelda II: The Adventure of Link. Even with the suspend feature of the 3DS version, the combat in this game is heavily weighted against you. Defending doesn't work against most things, enemy attacks are either completely untelegraphed, or telegraphed so quickly it's basically pointless, and enemies take a lot of hits to go down.
I suck at both crowd managing and action games, but as much as I love the writing and style the gameplay is relentless. Maybe I should bump it down from Easy...? (Yes, Easy. I'm terrible at this game.)

im terrible at real time strategy and this game is ridiculously complex; even with a guide, i have trouble keeping up.
I was able to beat the singleplayer campaign for Starcraft II on the hard difficulty setting, but the multiplayer is just beyond my level of skill. Even the bronze-class players are just too much for me to handle, and the demand for a very high level of micromanagement doesn't make matters easier either.
Resonance Of Fate. I got stuck on nearly every boss fight, and after chapter 4 or so I decided to call it quits.
Megaman X5.
Amusingly enough, I've beaten all four Zero games, which if I'm correct is considered the hardest series.
I should probably go back and try again, as it always bugged me that I never beat any of the X series.
Dark Souls, and probably by design as far as how I personally play games. I need a map in-game because my in-head mapping skills are horrible.
Speaking of the Megaman X series, I was never able to beat X2. I got stuck on the 2nd boss in Sigma's arctic fortress cause I couldn't defeat him fast enough before his "Wall tank" pinched me into a corner and destroyed all the floating platforms I had. I think the guy's name was Serges. It's rather funny too, because out of the three extra possible bosses, he's by far the easiest to beat when you first find him.
Devil May Cry 3 took me a very long time to get the hang of.
I like to keep my audience riveted.I couldn't beat X2 either but I usually never finish the non-Zero Megaman games since the last levels are just filled with insta kills and such. I got stuck on the jerk with the flail, that guy was brutal.
Actually I couldn't beat Fefnir's level in MMZ 2 with all the lava blocks. Yeah insta-kills in Megaman games suck.

Just floating around...Dark Souls: Keep rage quitting at the Crystal Caves. That place is pure unfairness. Will beat it one day.

Kings Quest Heir today Gone Tomorrow still gets me and of Course Zork, actually, classic adventure games in general, the only one I've legitimately beaten so far is Day Of The Tentacle
Apocalypse: Dirge Of Swans.Ugh, Magician Lord. That game was decent for a Neo Geo launch title, but ugh is it ever so unfocused in terms of design.

Mario 2 Lost Levels stands out in particular to me as being unfairly difficult.
You know, I have to wonder why Pit is obsessed with this site. It’s gonna ruin his life!There's a reason it's the only game in Super Mario All-Stars that lets you save after every level, instead of every world.
Phantasy Star 2 definitely feels like this for me, to where I shamelessly had to use a guide just to realize where I was supposed to be going. Not to mention the tisophone which lets you save anywhere safe instead of having to go to the data girl everytime. In fact I never finished it despite actually getting quite a bit easier/more interesting once you hit Dezolis and while I still have the file, I'm afraid I'll be too lost as to know what to do.
Similar stuff happened with Phantasy Star 1 and Shining In The Darkness as well, although it was more of me taking too long of a break and forgetting what I was supposed to be doing rather than difficulty(the former isn't THAT hard, the latter is incredibly grindy).
In fact a lot of older JRP Gs are like this(like late 80s/early 90s old), as they they're just much more obtuse in design and tend to be rather unforgiving/grindy.
