There's a few good scenes from Fringe that illustrate this concept.
I see someone get cut in half because he's pulling the plug of that device. While it's kind of badly drawn, I can still see the trope in it.

I have a message from another time...I really can't tell what's going on in that pic without looking hard, and it's pretty badly drawn to boot. There's got to be a better example out there somewhere.

I'm thinking of a two-panel mockup where the first guy is putting his hand through a portal but somebody trips over the power cord, then the second panel is him with no portal and a missing arm. "That could have been your head."

Looks like the guy gets mauled by whatever's behind the broken window. Nothing about it says "portal" to me at all.

There's a Friday The13th The Series example where a guy ends up embedded halfway in a wall because of this trope. Possibly confusable for another, but I'm not sure "the other" actually exists...
As in, that they are thrown through a wall? Do not recall such a trope, but it would not be this.
