◊? The characters are cousins Neji and Hinata from Naruto and the picture is a panel from the manga. Apparently, those are their alternate versions where Neji lusts after Hinata acts like a pervert towards her.
They will appear like that in the upcoming movie Road To Ninja too.

You know, I think maybe the best way to show this would be with family trees. Maybe show a family tree with two branches at the end (portraits of a brother and a sister) eying each other illicitly.

Frankly, I think the cousins one is quite good. The dude looks at her like he is about to jump on her in any second. And just the looks on the other characters' faces indicate that they're freaked out about it.

The issue I have with the Neji/Hinata one is that if I didn't know who they were, there's nothing to indicate incestuous attraction in the image.

Well that's the same issue with the Neji and Hinata one. A lot of people don't know who the characters are in these Marvel comics.
Maybe if we found two characters with similar features to make it clear they're related. Neji and Hinata do look similar, especially the eyes, but not in that picture.

My first thought about the Naruto pic was, "That's Neji, but who's the girl?" Even now when it's pointed out, I don't recognise her.
But all suggestions have the same problem. Just two people with (hopefully) some affection one or both ways, and a caption.

The title of the trope should provide enough context to establish that the characters in the image are related...now, if we can find a pic that makes it clear without having to resort to that, that will be preferred.

The twins from Monster. The family resemblance is not that strong in this one, but I still feel it illustrates the Trope well, even if it needs to rely on text to do it: [3]

With 18, I think a good caption to make clear that they are related (it's already implied in the similarity) would make them good. Not easily decided on which is better.
