3 katana attacks + 2 unarmed attacks probably means she has at least +11 BAB (for 3 primary weapon attacks in a full attack) and Improved Two-Weapon Fighting (for two bonus attacks). Note that you can "dual-wield" a regular weapon and an unarmed strike.

According to the relevant GiantITP thread, she does have a few monk levels stated here but the reason she can make five attacks in a round is because she has a high enough level to make three main hand attacks, and she took the Improved TWF feat, which gives her a second off-hand attack.

Worldbuilding is fun, writing is a choreSmite Evil does not indicate to you whether the target is Evil. You can use Smite Evil against anyone. It just doesn't give you any special damage bonus if used against a non-Evil target. And you don't have to Detect Evil before using it, but you do want to be sure your target is Evil because you only get so many usages of Smite Evil a day and if you waste it on a Neutral or Good target, well, sucks to be you. No refund.
When you use Smite Evil, it adds your CHA score to your attack roll and adds damage equal to your Paladin level to your damage roll, but only against an Evil target. A Good or Neutral target receives a standard attack and damage roll. When Miko dropped a Smite Evil on Roy, she was acting on the assumption that he was Evil based on her previous Detect Evil and thus expected it to kill him. When it didn't, that was weird and prompted a discussion of Roy's alignment.
Belkar and the rest of the Order were stonewalling her attempt to Detect Evil on Belkar from after that onwards, but she still seemed pretty certain and kept attempting to verify. When push came to shove, she went with her gut and burned a Smite Evil on him.
At no point is Miko actually fooled by the lead sheet. She just wants the proof so she can say, "I verified his Evil alignment before putting the fiend to death," and it won't be a lie.

And when she used detect evil on him in azure city, it was more of a way to locate him in the dark.

I'm not sure I do. I'm guessing they were for someone in her family and/or a slain soldier, but I wasn't sure if a specific known character was being implied.

Can't be; he is a creature that characters who are knowledgeable about ecology can identify, and while not native to the habitat that he was found in, he's most certainly not a god-eating abomination or people would have reacted differently.

One thing that struck me after reading is that unlike the OOTS, O-Chul knows Redcloak's personality and motivations pretty well and if'when he reconnects with them, he's a good source for that.
Because like I don't think the Order really has any knowledge of the "Goblin independence movement", do they? As far as they know, Redcloak is Xykon's head mook and brought an army of minions with him, right?
O-Chul is not only a much-needed walking encyclopedia on Team Evil, but a level head they desperately could use in general.
Also, the Go-set had me grinning with the warm fuzzies, too.

They'd probably note the Certainty Principle; if he doesn't reveal his theory, it's definitely true, but if he does, the effect of reader observation makes it definitely untrue.
Or it's just her being Lawful Anal, the rules say she has to get a positive Detect Evil, so without that, even if all other evidence makes clear he is evil, she can't act like she's proven he's evil and just make with the killing, simply for being 'evil'.
