  • Ensemble Dark Horse: Feinstone's patient Sarah is well-liked for being a Nice Girl and Action Survivor who gets a decent Black Comedy line while successfully begging for mercy.
  • Jerkass Woobie: Feinstone. He's not genuinely outright malicious, just very mentally ill. You can almost feel sorry for him in the second movie, since he's really trying to make an effort not to hurt anybody else.
  • Nausea Fuel: Basically everything Feinstone does to his patients through the whole movie.
  • Nightmare Fuel: As well as this.
    • Special mention should go to poor Bev's fate in the second movie. It's probably one of the most brutal deaths in either movie. Then we see her body after; the lower half of her face has no skin!
  • Retroactive Recognition:
    • A then-unknown Michael E. Rodgers played a minor role as a patient who was traumatized by Alan Feinstone's Ax-Crazy behavior and was nearly killed by him.
    • Mark Ruffalo also had a minor role in this film, as a flamboyant talent agent.
  • Special Effects Failure: Every time we see a close-up of a patient's mouth being tortured, it's an obvious model with rubber gums and tongue, and plastic teeth. Whenever a dental drill touches a tooth, that tooth flakes off, unlike real (hard) enamel and like (soft) plastic. In addition, whenever a cavity is shown, it's always a black "crater" on the surface of the tooth. This is completely unlike a real cavity, which starts as a thin channel (invisible to the naked eye) connecting the surface to the inside of the tooth, where the decay develops and eventually shows in transparence through the enamel. And whenever some teeth are removed from the fake mouth, subsequent shots of the real mouth of the victim show the teeth still in, either painted black in an attempt to hide them, or not even disguised at all.
