  • Bile Fascination: Considering that all of the individuals featured are morbidly obese, many are bed bound, and some even put this guy to shame, a lot of people tune in just because they can't believe people can get that big.
  • Harsher in Hindsight: Watching the episode of an individual who has since died since the episode aired, especially if their episode ended on a positive note.
  • Just Here for Godzilla: Many people tune in to see Dr. Now be a Deadpan Snarker to his patients.
  • Squick: Just seeing the condition many of the individuals are in can make you sick, never mind the way some of them eat in the beginning, which has included anything from five pizzas at a time to heaping plates of meat and other junk food.
  • Tear Jerker:
    • The death of Henry Foots, featured in season one. He lost the weight, married his on-again and off-again girlfriend and everything was looking up for him... until he died in May of 2013.
    • Kelly Mason was a young woman who had given up on life until deciding to make a final effort to lose weight. She had a successful surgery, lost over 300 lbs, and was shown reconnecting with her family as well as taking the first steps toward following her dream of attend college. Tragically, she died during filming due to a pre-existing heart condition. Dr. Now was visibly moved by her death, repeatedly stressing that she had done everything right throughout her journey, and it was All for Nothing in the end.
    • Sean Miliken entered the show at one of the highest weights, 1003 pounds. He endured a leg injury in high school that left him bedridden, a strained relationship with his father, and comments by fans about how his mother was an enabler, until his mother died midfilming, with Dr. Now noting he started putting on weight out of his previous habit of binge eating to deal with emotional problems. He survived less than a year after leaving the show.
