The plot of the album is:
Helena, a young woman, is murdered by The Mafia, or a like organization. The album's protagonist is the young man who was in love with Helena, the Deuteragonist is their childhood friend who mysteriously vanished for many years, only to show up at her funeral. As the protagonist declares that he wants to find the people responsible for Helena's murder and make them pay, the deuteragonist reveals why he's been gone so long: he's become one of the criminal underworld's top hitmen. With the deuteragonist as The Mentor for the protagonist's Naïve Newcomer, the two embark on a Roaring Rampage of Revenge, doing significant damage to the mob. Along the way, the deuteragonist meets and starts falling for a lonely, bored, discontented rich woman, a development the protagonist is not on board with as it compromises their mission. Eventually, our heroes are cornered, caught, and executed by the mob before even reaching the people ultimately responsible for Helena's murder.

  • Just what, exactly, is hidden in the bridge of Hang 'Em High? According to a Gerard Way tweet from 2013, it's a "subliminal buried story" by Keith Morris. Someone has managed to isolate the audio partially, but only the first part is really audible and the majority of it remains a mystery.
