The manga was actually the events of Natsuo's book that he revealed at the end. That's why it was filled with crazy things like stabbings, surprise pregnancies, magical comas, fights with gangsters, etc. Natsuo embellished what happened in order to make things more exciting. In reality the romance between Natsuo and Hina was far more mundane.
