- Easter Egg: The game's mock commercials advertise two songs by Jasmine Odyssey- Nice Smelling Face and Fish Whispers. If you dig into the game's options, there's an option to enable the full version of both songs in the game.
- Refitted for Sequel: Well, "Refitted for Spiritual Successor". The "Night Fever" and "Animal Magnetism" diseases were originally planned to be included in Theme Hospital.
- Likewise, the A Stitch in Time DLC allows one to time travel and theme their hospitals around several eras. The theme part was originally going to be a feature of the original game (hence the title).
- What Could Have Been: Two Point Studios wanted Steve Coogan to reprise his role as Alan Partridge as the radio DJ, and even used extracts from Mid-Morning Matters during testing, but couldn't get him due to scheduling conflicts.
