- Albert Throgmorton managed to survive a horrifying expedition to an undersea temple and made a daring escape when madness infected his rescuers. Then you slowly piece together it didn't save him as he was tracked down, murdered, and hung in a warehouse basement. Worse, you've been paid to find him by the victim's father who passionately believes he's still alive.
- Robert Throgmorton is a complete snob and a bigot against the Innsmouthers but his reaction to finding out his son died is a Little "No" and deny it.
- Rare for video game characters, Albert actually has a funeral held in his honor and you find women weeping at it beside a stoic Robert Throgmorton.
- Lewis Flynn describing the refugee situation of the Innsmouthers and their poverty after the destruction of Innsmouth. They may be fish monsters but it's a situation that all too many real people have to deal with. They needed food, medicine, and shelter.
- The discovery that the expedition members are still alive and trapped underwater. They've been driven hopelessly mad but you can rescue them (or leave them to die horribly in Cthulhu's temple in order to contain their madness).
- The Infected Men's Residence has an entire family slowly turned into monsters by the Wylebeasts sneaking into their house at night, including the children. The worst part is the children know this and are trying to warn their family but are ignored as imagining things.
- Sidney Stokes just wanted to raise enough money to bury his mother and turned to thievery. In the end, it got his friend killed and turned him into a murderer. It also can potentially end with his capture and death as well as robbery at Reed's hands.
- Joy Hayden used to be a glamorous nightclub singer but sharing the secrets of Oakmont resulted in her receiving a horrible curse of having her mouth sewn shut. Worse, Granny Weaver continues to harass her, gutting her dog and knitting a teddy bear out of its innards. Even after Reed banishes Granny Weaver, Joy can’t cut her punishment short and must endure it for two more years, for fear of retaliation from her fellow Oakmonters.
- Reed's hallucination of Graham Carpenter if you sided with Brutus. Unlike the accusatory visions of other people you killed, Graham's spirit (or whatever) hugs Reed and offers his mask, shocking Reed. It appears to be an act of absolution, driving home the point you, however indirectly, killed one of the nicest people in Oakmont.
