How Jigsaw gets his scars: The Punisher kicks him into a demonic-looking glass bottle crusher and as Billy curses at him, Frank turns the machine on, grounding Billy against the machine's walls with the crushed glass, smearing blood on the viewing ports. And during all this, he's screaming in pain and yelling for help. When he's later revealed to have survived, his face is briefly shown, caked in blood and glass shards; honestly, the way he looks in that scene is worse than he looks after getting his face "fixed"
Loony Bin Jim killing the orderly. Luckily, it isn't on-screen, but hearing the orderly's screams and the noise of his guts being torn out is still disturbing.
Before that, Jigsaw neck snapping an innocent doctor before entering Jim's room.
The sight of Micro's dead mother is incredibly disturbing, due to how her head has been blown off. The fact that the camera focuses on it prominently doesn't help.
