The movie depicts the emotional torment of Merrick's disease (everyone being scared of him, and some even treating him as subhuman) in great detail, but mostly glosses over the physical pain, even though you'd think it would be quite intense, due to (among other things) his back being covered in tumors and his head being so big his neck can barely support its weight.
