- When Ichigo returns from Soul Society, Benihime asks Ichigo why isn't he angry at Urahara for manipulating him and his friends or not giving him any information on Soul Society. Benihime is outright annoyed that Ichigo forgives Urahara and his utter lack of strategy. Since Benihime is Urahara's soul, that means deep down Urahara feels guilty for manipulating Ichigo and wishes the latter to use his head more.
- Ichigo doesn't question why Zangetsu isn't hollowfied when the Zanpakutou of the Visoreds are. However, given the sheer diversity in Zanpakutou spirits (for example Zabimaru can be a baboon-snake or two humanoids) he probably assumed it was just another case of diversity.
- Urahara literally threw away his research. Normally, wouldn't he go through it to see what is still viable in light of the new information? The Visoreds wound up in their state due to his research and shut away their Zanpakutou spirits for a century with him failing to help them. Urahara had an extreme reaction out of guilt and the realization he hadn't been working towards a solution at all.
- Hiyori is terrified of confronting and resolving things with her Zanpakuto/Inner Hollow and does her best to beat around the bush or stall Ichigo from helping her with it. Zanpakuto are reflections of one's souls; Inner Hollows are the worst parts of those souls. The Visoreds have had their Inner Hollows sealed for over a century and as demonstrated by Sakanade and Kinshara, they're not pleased about it. On Hiyori herself, she is 10 lb. of asshole in a 5 lb. bag on a good day. And her Inner Hollow has to be even worse than that. She doesn't want to admit it but she KNOWS resolving her issues with her Zanpakuto is going to go badly.
- Why does Ichigo's Inner Hollow ask to be called a different name when he's actually Zangetsu? Because Hakuran is smart enough to know where to take his losses and know not to currently push the issue. Hakuran is the only Hollow who gets along with his Host so he's not going to risk the freedom and acceptance he has by revealing that he's the real Zangetsu.
- Ichigo refers to having accelerated healing in Shinigami form. In canon, Shinigami are more durable and any rapid healing is due to kidou However, this is not necessarily a mistake on the author's part. Ichigo's information is limited to what he has found out and what has been discovered and given that a powerful enemy has taken interest in him, his teacher's priority isn't to tell "nope, that doesn't happen to everyone" and more "Okay, learn this so it can keep you alive". The rapid healing may in fact be a reference to the regeneration ability from his hollow powers. After all, Hakuran is the real Zangetsu so it is possible that the rapid healing is due to him even without Ichigo using any hollow powers.
