Josh
The main protagonist of the game, recently laid off from his job and on vacation in Romania with his pregnant wife, Kate. After surviving a train crash, he's forced to fight to save his own life and that of his wife's, who has been captured.
- Action Survivor: Before he comes to the mansion, he has little to no combat experience, yet he still manages to brave the horrors in order to save his wife.
- Drowning My Sorrows: Has an alcohol addiction, which causes issues within his marriage.
- Power Palms: Azoth, a seemingly arcane gauntlet with the ability to disrupt alchemical wards and One-Hit Kill almost every monster in Victor's castle. It shorts out after defeating Ernst, but Yeli is able to restore it after Josh gets his body back.
Kate
Josh's pregnant wife, and a central character in the game. Much of the game focuses on Josh's attempt to rescue her.
- Damsel in Distress: She is held captive by Viktor for most of the game.
- Imperiled in Pregnancy: Viktor deliberately targets her to extract her unborn child's O'Zhuvindo.
Viktor
A scientist with an interest in states of life and death, and the main antagonist in the game. He's responsible for the kidnapping of Kate and much of the deaths and transformations of the altereds.
- Bad Boss: He's sewn Marchosias's head to a dog's body to punish him.
- Creepy Crossdresser: Dresses up as his deceased wife, Mary, likely as a way of repressing his awareness of her death and his guilt over murdering her.
- Ink-Suit Actor: He's directly modeled after his voice actor.
Mary
Viktor's wife, and the true and final antagonist of the game. Hailing from a wealthy aristocratic family, Mary married Viktor and they had a child together. However, Viktor's obsessions with superhuman capabilities would spell tragedy for both Mary and their child.
- Dead All Along: Viktor strangled her after she blamed him for their child's death. This would lead to his desperate attempts to bring her back to life.
Yeli
A Romanian fortune teller and one of the passengers aboard the doomed train.
- Fortune Teller: Her main role, and much of her predictions at the beginning of the story are proved true by the end.
Ernst
Viktor's enforcer, a hulking juggernaut who smites everyone in his path in order to fulfill his master's orders. He is Yeli's husband, who was unable to escape Viktor's mansion and was turned into an undead monstrosity.
- Blade on a Rope: His main weapon is a pair of chains attached to his forearms with blades at tips he can send out to rip everything apart, including a few alchemical wards you encounter when Josh is in his body.
- The Brute: Ernst is Viktor's most physically imposing agent, usually sent to deal with serious threats.
- Implacable Man: He's completely impervious to gunfire and cannot be taken down by melee weapons. This is due to a regenerating alchemical ward placed on him that can be broken by the Azoth.
- Malevolent Masked Man: His face is usually covered up by an iron mask that can open to reveal his decayed face.
- Redemption Equals Death: His act of regaining control of his body and allowing Josh and Kate to get away angers Viktor who promptly executes him.
Marchosias
A man whose head is attached to a dog's body. He encounters Josh in the mansion and asks him to help him regain his original body.
- Faux Affably Evil: He is unfailingly polite to Josh while he's still bound to a dog's body, but once he reveals his true colors, he starts spouting threats.
- Evil Brit: He speaks in a refined British accent, subtly accentuating his depraved nature.
- Hoist by His Own Petard: He manipulates a zombified Jane to attack Josh, but once his control slips, she turns on him and immolates him.
- Mix-and-Match Critters: When Josh encounters him first, his head is attached to a dog's body as a punishment for failing Viktor.
