When repeatingly swinging it, the fourth swing and each of the following create a missile which hits a random target (or the player's target, if there's nothing else around). Since rapiers have a very fast attack speed, it's possible to wipe large groups of enemies by focussing on the toughest one while letting the magic missiles killing the surrounding mooks.
