** Those low growls emitting from the zombies and the fact that you're exploring nothing but wilderness or abandoned buildings doesn't make anything pleasant neither. In fact, most of the time one would be constantly and very nervously be watching their back throughout their surroundings when starting a session.
** The fact that the zombies are virtually relentless on their chases. As soon as they see or detect your presence, you better get a good start since they'll hunt for you in good fair distances... And it gets even worse when a full moon comes up in the 3.0 version and later. Zombies have deeper growls, actually pretty scary glowing red eyes, detect you from a much bigger distance and deal a lot more damage. What makes those eyes worse is that you can actually see them in the distance, and, sometimes, a zombie might have their back turned to you, only to turn around with those eyes glowing and scaring the shit out of you.

* QuicksandBox: You start with absolutely nothing and in the wilderness, consider yourself lucky if you can see some landmark, and the game world is filled with random junk and enemies who will easily splatter you. You know you need supplies and weapons to survive, but good luck figuring out where to get those.

** Creeping through a bunch of zombies in a town when you have no weaponry. It's already hard to fight one zombie with just fistsunless you have Overkill maxed out on Normal difficulty, whereas your fists hit hard enough to stun zombies, and that'll alert more as you kite it. Slowly making your way through the horde, as they moan, groan, and shamble about, is utterly tense. Gets even worse on Hard mode, since zombies are ImmuneToFlinching and the only way to reliably dispatch them in melee is with a Good or a jackhammer, both of which are extremely loud and capable of alerting zombies in a three-block radius.

* ScrappyMechanic: Drinks have to be cleaned with purification tablets. There's no way to restore them to a potable state like filter or boil them. This forces players to never get too far from a town with a drugstore, or give up at least eight inventory slots to lug around a generator and a jerrycan to activate drinks machines for soda cans (and the occasional bottle). It used to not be too much of a problem, as certain foods also restore thirst, but with the update that impedes players from eating when their hunger bar is full and diminish the results if the food restores more than necessary to fill the bar to 100%, it's a bigger problem. The More Farming GameMod mitigates this a bit with the craftable Charcoal, but it's only usable on the Dirty Water item that can only be obtained by fishing.

* DemonicSpiders:
** Flamer zombies. If one of those is in a mob, you can toss the idea of saving ammo by melee right out the window, as they explode in a huge fireball when killed, and if any regular zombie is caught in the blast, it'll become a Flamer as well.
** Wolves. They're tiny and don't have much health, but they're exceptionally fast (capable of outrunning a player going fast on a bicycle), attack just as fast and are ImmuneToFlinching. The worst thing about them by far, however, is their tendency to shove their heads inside the player's hitbox, so it's impossible to headshot one that's already attacking you unless you can leap on a ladder and away from it. They also deal a surprisingly large amount of damage to vehicles.

You take cover inside of a house, and realize that you have no idea where that guy is or if he has a bead on your head at the moment.

** Mega Zombies. Imagine a hulking beast that can resist multiple shotgun blasts, machine gun fire, and just about anything you throw at him, including whole cars, which he can also swat away without an afterthought, and you have a Mega Zombie. Even worse is that, from afar, they actually look quite similar to a regular zombie, making the possible encounters with them even more terrifying.
