Special mention to version 1.605, which fixed one minor bug while introducing a crapton of others, some of which were [[GameBreakingBug pretty awful]]. In this particular case, it was impossible to revert the update, because it changed the entire game's build (the "update" actually had you download the whole game again).
** Back in the first implementation of the Denshi Kairo dungeon, so many things were bugged that they had to take the server down the next day to ''revert the update''.
** Creator/AeriaGames's Game Masters (and even some Game Sages) would always claim that they tested content extensively before implementing. One would think "extensive testing" would make them notice that the most popular dungeon (Old Ichigaya Camp Gold) is lacking AI, or that the most rewarding cash run bosses (Shiva & Vishnu, which were being spammed to hell and back at the time) are lacking ''two thirds of their HP'', or that the soulstones they just implemented are not working at all.
** Atlus Online isn't much better. They apparently had no testers at all, and had to have the players test their contents. This was made rather obvious when they tried to fix the [[PlayerVersusPlayer PvP]] bug twice, but players confirmed they failed half an hour after the maintenances. They also needed player confirmation when the bug was actually fixed on the third try.

** Zhu Que's plugin: You need to go through a low level dungeon that gives no profit whatsoever in the hopes of finding the elusive special boss Zhu Que to get the key item and the earrings he drops, then create a dungeon on the other side of the map while having the earring equipped, and then talk to a statue in the boss room to trade the key item for the plugin. The problem here is that said special boss is [[LuckBasedMission so rare to spawn]] that some players have reportedly been running the dungeon ''dozens of times per week for over 3 years'', and still haven't seen the bird even once!

While this isn't a LuckBasedMission, as are most other plugins, once killed, Raphael will only respawn 8 hours later ''in real time'', making you need to wait there for hours if you don't know when was the last time he was killed. While this wasn't so bad back in 2009-2010, when we had 7 different channels/game worlds, now that we have only one server with one channel, it's awful with people {{Kill Steal}}ling most often than not, and some people who just kill him for the experience and drops.
** Nyarlathotep's plugin: You need to farm for a rare element, then trade it with an NPC for a consumable. Then you need to go to Nakano at a specific place at a specific time (between 0:00 and 6:00 during a full moon) and then use the item, which will spawn some DemonicSpiders. Once you defeat them, [[LuckBasedMission there's a chance the real enemy you need to defeat will appear]]. The main problem is that Nakano is a place with people passing all the time, so sometimes you'll get some random player wanting to KillSteal your plugin from you. It doesn't help that the game warns everyone in the area when the real boss spawns...

While this wasn't so bad back in 2009-2010, when we had 7 different channels/game worlds, now that we have only one server with one channel, it's awful with people {{Kill Steal}}ling most often than not, and some people who just kill him for the experience and drops.
** Nyarlathotep's plugin: You need to farm for a rare element, then trade it with an NPC for a consumable. Then you need to go to Nakano at a specific place at a specific time (between 0:00 and 6:00 during a full moon) and then use the item, which will spawn some DemonicSpiders. Once you defeat them, [[LuckBasedMission there's a chance the real enemy you need to defeat will appear]]. The main problem is that Nakano is a place with people passing all the time, so sometimes you'll get some random player wanting to KillSteal your plugin from you.
