* TearJerker: When Vito finds out that [[spoiler:his mother has died - while he's in prison, no less]], and [[spoiler:Marty's death - especially Joe's reaction]].
** [[spoiler:Killing Tommy Angelo from the first game, Henry dying horribly at the hands of the Chinese (because he was feeding info to the Feds - putting him in a Tommy Angelo situation), and Joe likely getting killed at the end.]]
** The pause screen music.

* NightmareFuel: [[spoiler: Henry being brutally murdered by the Chinese... who are using meat cleavers. The worst part is hearing Henry, who has spent the game up to this point cool and in control, screaming and crying in agony]].
** The slaughterhouse in the mission "Ball and Beans" is quite disturbing, too - especially since you can hear pigs squealing as they are slaughtered.

** According to Vito in ''{{VideoGame/MafiaIII}}'' [[spoiler:Joe managed to escape the hitman taking him in their car, and escaped to Chicago. Unfortunately, he was betrayed by the people he turned to for help, and was subsequently mutilated and murdered by them. Vito sends Lincoln Clay on a series of assassination jobs to take out the people responsible for it. However, despite Vito's story the game never truly confirms Joe whether Joe's alive or dead. Vito says he never found Joe's body and says he assumes Joe must be dead since he never got into contact with him. The biggest hint that Joe's still alive is at the end of the game, where Leo Gallante's driver briefly looks at the camera, and though he's in shadow, he looks near identical to Joe. So Joe may literally just be hiding.]]

* BolivianArmyEnding: [[spoiler:The ending of the game is ambiguous enough that it's unclear who exactly lives and dies. The game seems to imply that Joe dies but Vito lives, but it's also entirely possible that Joe and Vito would survive to take on the remnants of the Vinci family after already previously severely wounding the others. It's also entirely possible that the game has an outright downer ending where Joe and Vito have outlived their usefulness to the Vinci family, with the two of them being unceremoniously killed so the Vincis can rise to power due to no real opposition anymore.]]
** The sequel confirms that Vito survives, and is living in New Orleans nursing a grudge against the Italian mob.

* ContestedSequel: Given that ''VideoGame/MafiaTheCityOfLostHeaven'' is a CultClassic, this was pretty much a given. While it was liked well enough, many people criticized it for it's unsympathetic characters, simplified gameplay and lack of Free Ride Mode, making it extremely short and without any real replay value.
