Shao Kahn, furious with the failure of his servant Shang Tsung, was [[YouHaveFailedMe about to end]] Shang Tsung's life. But Shang Tsung came up with an idea: since the invitation to the Mortal Kombat could not be refused, if the Earthrealm warriors were invited to a tournament in Shao Kahn's realm Outworld, [[ButThouMust they would have to come]]. There, Outworld warriors assembled by order of Shao Kahn could kill them and conquer Earthrealm itself. Shao Kahn liked the idea, so he restored Shang Tsung's youth. Meanwhile, a Shokan named Kintaro learned of the apparent death of Prince Goro in the Earthrealm tournament, and swore revenge on the Earthrealm warriors who killed him.

The game ran out with the blood, by adding an extra Fatality to their characters, and more Stage-specific Fatalities. It also dropped the score system, replacing it with the "Wins", and added two new {{Finishing Move}}s: the Babality, which [[ExactlyWhatItSaysOnTheTin turns the opponent into a baby]], and the Friendship, a NonLethalKO, which had the characters doing friendly things, possibly in order to please the MoralGuardians who were disturbing during the time of the first game.
