* HarderThanHard: Ranger Mode. It doesn't matter whether you pick [[HarderThanHard Ranger Easy]] or [[BeyondTheImpossible Ranger Hardcore]], it's still quite difficulty and the game was NintendoHard to begin with. A list of the notable things it does to murder players includes:
** Making ammunition incredibly scarce to the point where you will likely go through most of the game with only your knife.
** Making Artyom more fragile so that even a few hits from a Nosalis can put him in critical condition.
*** And every single hit seems to go to your gas mask, so those break more quickly too.
** Making enemy attacks do more damage.
** Fighting humans is so difficult as to be nearly impossible.
*** Seriously, the best way to deal with gunfights is to evade everyone.

Whilst there are a hefty amount of enemies to get through, especially compared to the rest of the game, and it's very practical and satisfying to burn through the last of your [[TooAwesomeToUse military-grade ammunition]] there, if you look closely [[spoiler: most of the mutants surrounding you are [[AvertedTrope actually only 2-D, have no way of attacking you and are only there to make it look like there are hundreds of enemies surrounding you.]]]] However, at the point where it happens you're supposed to be running very fast and if you dally too long in the section noticing this you get your face ripped off, it doesn't make too much difference.

There's one tunnel that's so haunted that even the mutants and rats are too scared to go there; Khan comments "This tunnel must re-live its past. And those unfortunate enough to walk here at such a moment, usually join that past." There's one very creepy scene where Khan chants at a bunch of ghosts so that they let you through.

** 4A Games team made an armor attachment system that worked just [[GoneHorriblyRight like it should]]: Every pouch, helmet, magazine or armor plate honestly did it's job and substracted a certain amount of damage dealt by a projectile that hit them. But it was never explained to the player, which led to mass confusion and complaints that enemies are literal bullet sponges, while all it took was a single bullet to the neck.
