** The Rodriguez gang in Sedit hates ghouls, to the point of harassing a local ghoul-run inn and threatening to murder them.
** The Mutant Hunters kill all mutants on sight. This doesn't just involve ghouls and super mutants, but also regular human beings with minor mutations such as an extra toe.
* ForeverWar: The town of Rathole (formerly Kirtland) has been caught in a war between two rival gangs for years.

* KingmakerScenario: The main character can break stalemates between local factions in two towns: Rathole and Corath. Of course, there's nothing preventing them from just killing off both factions.
* MorallyAmbiguousDoctorate: The clinic in Albuquerque's Dead Quarter is filled with these. The doctors kidnap tribals and poor people and forcibly extract their organs in order to satisfy the demands of their rich patrons.
* PostApocalypticDog: In true Fallout tradition, a dog can be recruited as a party member.
* PromotedToPlayable: Keri, the daughter of the Crimson Caravan boss who was just an NPC to talk to in VideoGame/Fallout1, becomes a party member in the mod.
