When Peter comes to the Drunken Clam deprived of sleep from Brian talking to another dog all night, the guys suggest he try some Red Bull as is given a free can to try it out. Peter soon becomes addicted to the energy drink and it annoys his fmaily so much, Lois empties all of Peter's cans. In response, Peter decides to create his own energy drink and despite Brian's warnings not to drink it as it contains kerosene, Peter drinks it and instantly collapses. Dr. Hartman says that the kerosene caused both of Peter's kidneys to die and is in need of a transplant and that he has to undergo dialysis treatment until a donor can be found. Peter decides to skip dialysis to watch TV sith the guys, which causes his skin to turn yellow and vomit blood and passes out. While Peter recovers in the hospital, Brian is found to be a matching donor for Peter, but Brian has to give both his kidneys to save Peter, which would mean he would die. Brian decides to do it anyway as a way of paying Peter back for the years he took care of him. Stewie discovers what's going to happen and not wanting to lose his friend to his idiot father, he kidnaps Brian and takes him to his "house" on a playground. Brian tells Stewie that if he doesn't let him go, his father will die and Stewie breaks down crying saying he doesn't want to lose anybody. The day of the operation, just as Peter and Brian say their final goodbyes, Dr. Hartman comes in saying he's a match for Peter and will donate his kidney to him, since dog kidneys won't save Peter.
Meanwhile, Chris wants to write an essay for an upcoming assembly that will introduce a special appearance by president BarackObama. Meg gives him some tips about hope to inspire him, but instead he copies her words into his essay and it wins the contest. When Meg complains about it, Chris denies it saying she technically didn't write it, but after Peter and Brian's ordeal, they make up and share credit on the essay and the two recite it at the assembly as their family watches. The episode ends with President Obama singing "Honesty Sincere" to a crowd of swooning girls.

When Peter comes to the Drunken Clam deprived of sleep from Brian talking to another dog all night, the guys suggest he try some Red Bull as is given a free can to try it out. Peter soon becomes addicted to the energy drink and it annoys his fmaily so much, Lois empties all of Peter's cans. In response, Peter decides to create his own energy drink and despite Brian's warnings not to drink it as it contains kerosene, Peter drinks it and instantly collapses. Dr. Hartman says that the kerosene caused both of Peter's kidneys to die and is in need of a transplant and that he has to undergo dialysis treatment until a donor can be found. Peter decides to skip dialysis to watch TV sith the guys, which causes his skin to turn yellow and vomit blood and passes out. While Peter recovers in the hospital, Brian is found to be a matching donor for Peter, but Brian has to give both his kidneys to save Peter, which would mean he would die. Brian decides to do it anyway as a way of paying Peter back for the years he took care of him. Stewie discovers what's going to happen and not wanting to lose his friend to his idiot father, he kidnaps Brian and takes him to his "house" on a playground. Brian tells Stewie that if he doesn't let him go, his father will die and Stewie breaks down crying saying he doesn't want to lose anybody. The day of the operation, just as Peter and Brian say their final goodbyes, Dr. Hartman comes in saying he's a match for Peter and will donate his kidney to him, since dog kidneys won't save Peter.
