* The Ghost Pepper gives you a chance to shoot blue flames, similarly to the Blue Candle active item, the chance of which scales with luck. On its own, it is nice but not too overpowered. * The D1, Isaac's Greedier Mode unlock, has the simple property of duplicating a random pickup in the room. In addition to all the normal pickups, this includes chests. However, if you remove all but one pickup from a room, then the D1 will duplicate it without fail. This essentially allows you to mimic the functions of the Blank Card and Placebo in a single item, albeit more tediously, or spawn as many chests as the floor will allow. Mix it with the Car Battery and a battery consumable to create infinite batteries and thus infinite charge for a floor, since it will create two batteries for every one consumed.

Ipecac causes them to explode, Techology and Jacob's Ladder let them shoot towards enemies instead of just dealing contact damage... and then there's the lynchpin, its interaction with The Forgotten. Needless to say, picking this item up is always a truly massive boost to your DPS.

However, what it also does is mark enemies and projectiles, and once the invulnerability ends, you blitz through everything you touched, gaining increasing damage buffs for everything hit. With practice, you can exploit enemy attacks to become immensely powerful, to the point where bullet-spamming bosses like Hush and Mega Satan can be completely decimated if you take 20-30 shots in that brief period. In addition, the shadow strike scales off of damage, so if you manage to find Ipecac, your melee strikes can shred some bosses in a single hit if a few bullets are tagged first. It's very easy to practice with as well, as Tainted Judas starts with it in every run.

* Binge Eater is regarded as a very strong run-winner. Exclusive to the Secret Room item pool, this item makes every item pedestal in the run swap between its normal item, and one of eight of the red heart-granting food items. However, in addition to the heart container each item normally grants, each food item you pick up ''also'' heals you for three red hearts instead of one, gives you two permanent stat-boosts (dependent on which item it is), and grants a temporary damage increase (for all foods). This allows you to convert any disappointing or otherwise useless things (such as active items when you've settled on one) into something highly beneficial, without losing out on the potential for getting other good items due to the item-switching mechanic.
