The answer is actually pretty simple - lava is molten rock, and in the Overworld it's just about broken the melting point so it's still in a semi-liquid state, taking more effort to flow. The Nether on the other hand is a hot hot place, melting the lava even further with its high temperatures and thus making it completely liquid, allowing the lava to spread very easily due to its consistency being much closer to water.

* Similarly, if you accidentally (or otherwise) attack and/or kill a Villager, the fastest way to counter the resulting reputation drop and increased prices is to trigger and beat a Pillager Raid. If this becomes a recurring thing, they would probably catch on to what you're doing, but decide that forgiving you afterward regardless is preferable to risking another Raid.

Most likely because they deal with a infinite amount of something, arrows for the former and durability for the latter, if there are two enchants that deals with this on one weapon, there might be major cataclysmic events (e.g. the enchants causing the weapon to collapse into a dense singularity that might turn into a black hole due to the enchants generating so many unneeded excess), so the character is wise enough to refuse doing the combination!
