As an interesting variation on a recurring character throughout the series, Robbie the Rabbit is fought in this game as an enemy, appearing in groups all throughout Lakeside Amusement Park as the most seen enemy of the area.

* AscendedExtra: From a minor RunningGag of sorts spotted throughout the series to finally becoming a fully hostile entity in ''The Arcade'' as a monster type seen during the Amusement Park section.
* ChainsawGood: Some of them have chainsaws that they'll swing at Eric and Tina for ''massive'' damage.
* ColdSniper: Some of them have hunting rifles and will position themselves away from the player to shoot at them.
* {{Expy}}: Throwing hatchets and wielding chainsaws while they spread around the park and flank the player makes them act not too dissimilarly to [[VideoGame/ResidentEvil4 Los Ganados]] if they had access to rifles.
* HellIsThatNoise: This version of Robbie tends to announce their presence with a high-pitched, cartoony laugh that sounds forced and disturbing. Killing them makes them let out shrill screams before they melt.

The second boss of both the game itself and the Brookhaven Hospital section. A giant winged monster found at the hospital roof in the Otherworld, it's keeping Emilie under its protection.

* AntiVillain: The game leads the player to believe the creature is keeping Emilie hostage as the other bosses do to Eric's entourage, but Tuberculosis is actually protecting her from the other monsters by keeping her at the roof away from everything else.
* AttackItsWeakPoint: A grown, exposed tumor that looks like a beating heart, which it often tries to cover so the player won't shoot at it for massive damage.
* BladeBelowTheShoulder: Its right hand is overgrown and shaped into an actual axe blade that it can swing to deal damage.
* BossRoom: The Otherworld Brookhaven Hospital's rooftop.
* DemonicHeadShake: Often. It also extends to its arms on occasion.
* GetBackHereBoss: Hurt it enough and it will [[{{Flight}} take to the air]], trying to pull a surprise attack on Eric and Tina as it hides in the red fog surrounding the roof. It's possible to stop the attack from coming by shooting just as it's about to land.
* MeaningfulName: Tuberculosis is a potentially lethal and airborne lung disease that is also dangerously contagious. The monster itself is also potentially lethal and airborne, not to mention being fought at a hospital, a place where diseases such as tuberculosis would be treated.
* OurDragonsAreDifferent: It has some draconic traits to its design, such as the muzzle, a tail and wings.
* RuleOfSymbolism: The monster is named after a lung disease that might cause a build-up of blood or fluid in the lungs, effectively leading a person to drown, similarly to how [[spoiler:Hanna herself]] drowned in Toluca Lake. The creature's design also brings to mind a fairy tale, acting as a combination of both the heroic knight and the monstrous dragon, coming to Emilie's help and defending her from other monsters.
* WingedHumanoid: It has two flaps of skin that extend from its arms that function as wings. Despite its size, those things ''do'' allow it to fly.

A giant doll-like monster that drags Eric's friend George into the darkness in an attempt to kill him at a [[VideoGame/SilentHill2 familiar underground graveyard]].

* AttackItsWeakPoint: Its head and navel take the most damage.
* BattleThemeMusic: "Until the Stars Go Out", the same theme used in ''VideoGame/SilentHill4TheRoom'' for the final boss fight against Walter Sullivan. Ironically, Mama is fought at the same place where Walter's grave is first seen.
* BossRoom: The graveyard section of the Labyrinth, previously seen in ''Silent Hill 2''.
* CeilingCling: It will often get on the ceiling and crawl around it facing downwards so it can spew out Creepers at the player.
* CompositeCharacter: Invoked. Its face looks like a mixture of both Emilie and Hanna's facial features.
* CreepilyLongArms: Creepily long ''forearms'', woodened and extending like they're stripped to the bones. The legs are pretty much the same.
* CreepyDoll: Unlike the usual examples of this trope, this doll is ''a giant'', towering over the player as it attacks them. Not only that, it has creepily thin arms and spikes all over its body.
* FlunkyBoss: It can spew out Creepers from its mouth.
* LightningBruiser: It's ''fast'', able to go from one part of the cemetery to another in seconds while crawling, dealing a lot of damage if it hits the player.
* MarionetteMotion: It moves like one at first with its lanky, jerky movements. When its health is low enough, it drops that so it can crawl at top speed across the terrain.
* RuleOfSymbolism: The creature's face is a mixture of both Emilie and Hanna's features, hinting that the two have a connection. The name "Mama" and the lanky arms might be an allusion to [[spoiler:Hanna's mother and how she held up her daughter to throw her into the lake, reflected later when Lorraine's spirit does the same to Emilie with creepy bony hands of her own]]. The ability to spew Creepers might be a nod to "spewing poison", ie saying harsh, hurtful things under the guise of something more pleasant. The doll-like shape and the fact that it looks adult could also be a reference to [[spoiler:Hanna's]] resentment over not being able to grow up, since dolls are typically toys made for young girls.

* FlunkyBoss: The Phantom will summon about 8 bony hands from the water to attack the player. In order to achieve the Good Ending, the hands must all be shot until there's none left. Not doing so earns the Bad Ending.
* LosingYourHead: It's a monstrous version of Hanna that is only comprised of her demonic-looking head.

* RuleOfSymbolism: It's a physical embodiment of Hanna's pain and suffering in the water both while and after she drowned. The hands clinging to the creature are a reference to another cutscene where a group of these hands drag her into the depths, symbolizing her drowning and the paranormal influence that caused the sinking of the Little Baroness. The huge arm coming out of her mouth is a physical representation of her desperate cries for help that were never heard, and her struggle to try and get out of the water without success.
