Thanks for showing more examples, but now I'm even more undecided.... so what I suggested is to be a mix TeleportersAndTransporter, LiterallyShatteredLives, TheWormThatWalks, NinjaLog, PullingThemselvesTogether... I still think we should make this a new trope, probably a subtrope of PullingThemselvesTogether, or maybe sister trope since this is about shattering into pieces then reassembling, instead of merely shattering or merely reassembling. Or super trope, maybe?

And yes, Arivne, the example with Mina Hakuba is exactly the same as what I want :) so it fits anyway. So yeah, grouping it with VillainTeleportation isn't really accurate. Besides, is {{Madonna}} a villain? XD

Wait, I just found a super trope, LivingApart, but I don't think any of its current subtropes fits this. For one, I'm not looking for AppendageAssimilation (which is about adding foreign objects to your body), and DetachmentCombat doesn't really fit the purpose. Now I really am suggesting this to be made a subtrope under LivingApart :)
