* The ''{{Scrubs}}'' episode "My Screw Up" features this for Perry Cox. During the episode, Cox goes home at the end of a shift, leaving J.D. to take care of a patient. When he comes in to work the next day, J.D. tells Cox that the patient had died. Cox angrily blames J.D. for the mistake, but also blames himself, thinking that the death wouldn't have happened if he hadn't left. He decides to stay at the hospital around the clock in a bid to prevent any further "screw ups" to cause more people to die. The entire episode he's accompanied by his brother-in-law Ben, who repeatedly tries to get Cox to forgive J.D. and himself, and to go home and get some sleep. By the end of the episode, though, in a heartbreaking twist, it's revealed that [[spoiler:it was actually Ben who had died. Perry had hallucinated him throughout the episode, out of a combination of sleep deprivation and grief]].
