Getting hurt means actual injuries that can take weeks to heal (assuming you don't die before receiving medical attention) and death means someone has to take care of the resulting corpse. Lastly, the players no longer have inventories to hold all their supplies and shops are limited to what's actually present, which results in the players quickly running low on supplies.
And on top of that, Halkagenia isn't happy with all these monsters suddenly attacking the countryside from a flying castle the size of a mountain, which leads to serious tensions between Aincrad and Halkagenia.
On the flip side, the story also has one of the most heartwarming interactions between Louise and her mother I've ever seen. Louise is expelled from the academy for apparently not having any magic, loses her cloak that declares her a noble, and returns home fully expecting to be executed for "pretending to be a noble". When she arrives completely resigned to her fate, Karin tells her "I have three daughters. Do not make me a liar." as she puts her own cloak on Louise's shoulders.
