Does the Madoka Magica example really belong on here? Everything indicates production for that series went smoothly, with a delay due to being Too Soon. The actor's reactions don't seem to count eithier, considering some of the otber examples.

It kinda does sound worthy of this trope, to say nothing about Urobuchi's initial reluctance to continue the series after Rebellion. Feel free to specify anything that would counter what was written so far.
