You know, I actually kind of wonder if the Pokemon thing with Porygon is actually an example. Obviously the creators of the show are aware that the fictional characters aren't responsible for the animation effects used on the show. They didn't keep the Porygon line out of the show because they blamed it for the seizures, they did so because having it show up again would remind people of the events because the episode heavily featured Porygon. Pikachu's attack was the direct cause of the seizures, but because he'd been featured in the entire show before then, Pikachu wasn't as easily associated with the incident as Porygon. (Of course, the marketing concern probably played a role as well.)
