I'm pretty sure the CSI example is wrong. I have no trouble remembering episodes with the team investigating kidnappings and robberies all the way back to season 1, not to mention a lot of apparent murders that turned out to be accidents or suicides at the end. CSI:Miami, on the other hand... could be summed up as "It is always murder and Horatio always gets the perp".

If CSI only averted it once, then presumably the "Grave Danger" two-parter doesn't count.
