This seems to happen to your soldiers in XCOM2 (if they're lucky enough to survive the grievous injury they just took).

I believe Fable 2 had a scar system like that!
I swear I remember there being a videogame where being KO'd in it would leave a permanent scar of some kind. Similar to the "injuries" thing in the Dragon Age series, only permanent, and probably cosmetic?
