Spoiler Policy explicitely say that the "moments" pages (Awesome, Tearjerker, etc) and yes, Nightmare Fuel, are Spoilers Visible by nature

That's for the text and examples. The image is part of the description, and the description is always spoiler-free. That said, poor Vision was in the trailers.

Images tend to draw peoples' attention (they are even shown on our Google search results), Most peoples' eyes are immediately drawn to images anyway.
Since this is, you know, Image Pickin', I would think that when you pick an image, the policy for choosing images would be the guideline to picking an image over the general spoiler handling guidelines.
