Unfortunately no. The wikia page I got it from links to a Deviant Art entry, which links to a blog post that simply compiles transformation sequences from the work without mentioning which issue they're from.

Not necessarily. The core of the trope is that each of the ensemble represents classical elements, and while it's typically only 4 (fire, earth, wind, water), it's not limited to this number. The description mentions that the team may include a fifth element (lightning, aether, etc.), usually serving as The Heart.
It really shouldn't be, the trope is heavily related to the Four-Temperament Ensemble. It should not be a group of 5 unless it is a Sixth Ranger situation like Sailor Moon.
A Five Element Ensemble including The Heart is something different.
I think you're reading into the trope name way too far. There's no reason why this trope has to be dependent to Four-Temperament Ensemble.
The description say that the two may correspond, but both the write-up of the trope and the examples emphasize more about the fact that each member of a group/ensemble represent a specific Elemental Powers that differ from each other, rather than the actual numbers of the ensemble.

@13: I think the Avatar suggestion got downvoted because at wiki size, the pic doesn't actually show the elemental powers as clearly. I can only see the fire, while the rest looks like they're just posing.

Hmm, just noticed that Elemental Powers has the same team illustrating the page. I think we should try to find a replacement there that places less emphasis on the Ensemble aspect.
