I think it's clear enough that the guy has taken the other's eyeball and is licking the blood off of it, but what's not clear to me is if he's dead or just standing there looking vacuous. The caption is doing all the work on that front.

I think it works but I can't take it seriously with Bakura looking like a kitty licking food.
^^^ That's a human eyeball? I think it looks more like a talisman or a gem that he took from someone. The hair sort of hides the wound, and I had to focus for a bit to tell that they're even different people; black and white has that effect.

It's the Millenium Eye (so not actually a real eyeball). Of course, it was still in Pegasus's eye socket.
That said, the first panel doesn't add much and could be removed.

I tried hard to find an image from Yu-Gi-Oh! to go on Monster.Yu Gi Oh. This is the best I could find.
