My first choice is definitely Captain America vs. the Winter Soldier.
"Hand to hand" as in not including weapons at all?
Because although it starts with gunfire and later there's some Improvised Weapon use, the action during the "Freebird" sequence of Kingsman: The Secret Service was mostly melee combat, and all in all pretty damn awesome.

The way each hit syncs with the music is great.

Can't see it due to specific country copyrights. But yes, that fight from They Live is really cool.
And seriously, the kitchen fight from The Raid 2 deserves further praise.
How long did it take to properly film that awesomely choreographed sequence?
The choreography's great, but did they go seriously overboard on the effects or what?
What do you mean by effects? The blood squibs?
The "whoosh" sounds and meat tenderizer.
Personally I actually felt that those effects enhanced the fight scene.

Like the title says, what do you guys think are the best hand to hand combat fight scenes you've seen in films? I'll start with my personal favorite:
Yep, still nothing beats the Neo vs. Smith fight from The Matrix imo, and one of the reasons why it is still my all time favorite movie (screw the Nostalgia Critic's video XD). The fight choreography, acting, emotions, cinematography, and basically everything about it is pretty much perfect.
